In order to get screened for an appointment with me, I require that you either supply a previous provider reference, employment reference, or perhaps register with a third-party screening service, i.e. Preferred411 or Date-Check (both are highly recommended)*.  A photo I.D. will be required upon arrival.  Also, you can find a link to both of the above mentioned sites on my "Partners" page.
